The Devlopment of Reflexive Anthropology Reflexive anthropology has pressured scholoars to recognize their own biases and look increasingly inwards when studying ââ¬Å"otherâ⬠cultures. Reflexive anthropology is a break away from the traditional study of a clearly defined ââ¬Å"usâ⬠and ââ¬Å"them,â⬠that seeks to shift towards indentification rather than difference. It attempts to uncover the politics behind ethnography. Reflexivity shows how ââ¬Å"weâ⬠are effected by ââ¬Å"othersâ⬠, and how ââ¬Å"othersâ⬠are effected by ââ¬Å"us.â⬠It holds anthropologists accountable for what they write, and how they represent culture. In realizing the ways that they had imposed upon her, Kondo realized how too often academics impose upon the people that they are studying. She says that ââ¬Å"In searching for structure to impose on experience, one all too readily reduces the complex to the readily apprehensible, assuming authority and control over oneââ¬â¢s informants to make them fit into oneââ¬â¢s own categories, according to oneââ¬â¢s own motivesâ⬠(82). Kondo argues that writing ethnography is a way for the writer to ââ¬Å"reencounter the other ââ¬Ësafelyââ¬â¢.â⬠Going through the entire process of her experince with the informants allows the author to place herself into the text, and allows her to address herself. Encourpoating the ethnographerââ¬â¢s personalized experiences into the writing puts the person at a certain level of vulnerability. They are, in many ways, just as vulnerable as the people they are studying when so much of their personhood is included into the book. However, such notions cannot be taken too far. It is dangerous for an author to believe that s/he can fully understand how their informants think and feel just because they experience similar feelings, such as vulnerability. The airplaneââ¬â¢s engines also separated in flight and were found several blocks north and east of the main wreckage site. All 260 people aboard the airplane and 5 people on the ground were killed, and the airplane was destroyed by impact forces and a post-crash fire. Flight 587 was operating under the provisions of 14 Code of Federal Regulations (CFR) Part 121 on an instrument flight rules flight plan. Visual meteorological conditions prevailed at the time of the accident. The accident airplane was delivered new to American Airlines on July 12, 1988. At the time of the accident, the airplane had accumulated 37,550 flight hours and 14,934 cycles. History of Airbus A300 The development of the A300 airplane began in May 1969, and the first flight of an A300 occurred in October 1972. The A300B2 and A300B4 models entered service in May 1974 and June 1975, respectively. The development of the A300-600 series airplane (a derivative of the A300B2/B4) began in December 1980, the first flight of an A300-600 occurred in July 1983, and the airplane was certificated in March 1984. Before the accident, 242 A300-600 series airplanes were in service worldwide. The A300-605R is one of several variants of the A300-600 series airplane. The ââ¬Å"5â⬠refers to the type of engine installed on the airplane and the ââ¬Å"Râ⬠refers to the airplaneââ¬â¢s ability to carry fuel in the horizontal stabilizer. (National Transportation Safety Board [NTSB], 2004, p. 14) First Officer Information The first officer, Stan Molin age 34, was hired by American Airlines in March 1991. He held an ATP certificate and an FAA first-class medical certificate dated October 18, 2001, with a limitation that required him to wear correcting lenses while exercising the privileges of the certificate. The first officer received a type rating on the A300 in November 1998. According to American Airlines records, the first officer had flown Shorts 360, Beechcraft 99, and DeHavilland DHC-6 airplanes in commuter and regional operations under 14 CFR Parts 121 and 135. He had accumulated 3,220 hours total flying time in commercial and general aviation before his employment with American Airlines. American Airlines records also indicated that the first officer had accumulated 4,403 hours total flying time, 26 including 1,835 hours as an A300 second-in-command. (National Transportation Safety Board [NTSB], 2004, p. 1) Rudder Structure The A300-600 vertical stabilizer and rudder were constructed with composite materials, that is, mixtures that contain two or more distinct materials that are unified into one combined material. (NTSB, 2004, p. 15) Composite materials Carbon fiber is a form of graphite in which these sheets are long and thin. You might think of them as ribbons of graphite. Bun ches of these ribbons like to pack together to form fibers, hence the name carbon fiber. These fibers arent used by themselves. Instead, theyre used to reinforce materials like epoxy resins and other thermosetting materials. We call these reinforced materials composites because they have more than one component. Carbon fiber reinforced composites are very strong for their weight. Theyre often stronger than steel, but a whole lot lighter. Because of this, they can be used to replace metals in many uses, from parts for airplanes and the space shuttle to tennis rackets and golf clubs. (Carbon Fiber, 2005, p. 1) Rudder Structural Analyses NASA-Langleyââ¬â¢s and Airbusââ¬â¢ analyses determined that the fracture of the right rear main attachment lug was the most probable initial failure. The analyses indicated that, The vertical stabilizer fractured from the fuselage in overstress, starting with the right rear lug while the vertical stabilizer was exposed to aerodynamic loads that were about twice the certified limit load design envelope and after the right rear main attachment lug fractured, all of the remaining attachment fittings would fracture with no increase in external loading. (NTSB, 2004, p. 115) Flight Control System The A300B2/B4 model used a rudder control system employing a Variable Lever Arm (VLA) to limit rudder travel. A similar rudder-ratio changer design is also found in most other transport category aircraft. The VLA limited the amount of rudder available to the pilot as the airplaneââ¬â¢s speed increased. The rudder pedals consistently moved the same physical distance, yielding a proportion of rudder relative to speed. In 1988, Airbus implemented a completely new rudder design, which significantly modified the function of the previous model and hence, the handling qualities of the new A300-600 airplane design. This new system used a variable stop actuator (VSA) which is also found in the MD-80. The VSA also limited the amount of rudder available to the pilot. The difference in this system is that the distance which the rudder pedals moved also decreased as the rudder movement decreased in proportion to speed. A significant flaw in the design failed to offer the same kind of protection as in the McDonnell design. The MD-80 limits rudder travel and affords protection in the form of rudder ââ¬Å"blow downâ⬠should an operator demand more rudder travel (with resultant excessive load) than the structure can withstand. These kinds of redundant system designs are common in commercial aviation. The Airbus Flight Crew Operations Manual (FCOM) addresses the rudder system much like any other manufacturer and, in fact, did not change the language of the FCOM even after changing the A300 design from the VLA to the VSA system. (Allied Pilots Association, 2002, p. 9) Blow-Down System On an aircraft equipped with a hinge moment limiting (or ââ¬Å"blow-downâ⬠) system, a device is employed to limit the force capability of the hydraulic actuators, and thereby aerodynamic forces limit the maximum rudder deflection output as airspeed or aircraft configuration changes. American Airlines, 2004, p. 11) Vortices and Wake Turbulence Vortices form because of the difference in pressure between the upper and lower surfaces of a wing that is operating at a positive lift. Since pressure is a continuous function, the pressures must become equal at the wing tips. The tendency is for particles of air to move from the lower wing surface around the wing tip to the upper surface (from the region of high pressure to the region of low pressure) so that the pressure becomes equal above and below the wing. In addition, there exists the oncoming free-stream flow of air approaching the wing. If these two movements of air are combined, there is an inclined inward flow of air on the upper wing surface and an inclined outward flow of air on the lower wing surface. The flow is strongest at the wing tips and decreases to zero at the mid-span point as evidenced by the flow direction there being parallel to the free-stream direction. When the air leaves the trailing edge of the wing, the air from the upper surface is inclined to that from the lower surface, and helical paths, or vortices, result. A whole line of vortices trails back from the wing, the vortex being strongest at the tips and decreasing rapidly to zero at mid-span. A short distance downstream, the vortices roll up and combine into two distinct cylindrical vortices that constitute the tip vortices. The tip vortices trail back from the wing tips and they have a tendency to sink and roll toward each other downstream of the wing. Again, eventually the tip vortices dissipate, their energy being transformed by viscosity this change may take some time and may prove to be dangerous to other aircraft. The tip vortices cause additional down flow (or downwash) behind the wing within the wingspan. For an observer fixed in the air, all the air within the vortex system is moving downward (called down wash) whereas all the air outside the vortex system is moving upward (called up wash). An aircraft flying perpendicular to the flight path of the airplane creating the vortex pattern will encounter up wash, downwash, and up wash in that order. The gradient, or change of downwash to up wash, can become very large at the tip vortices and cause extreme motions in the airplane flying through it. An airplane flying into a tip vortex also has a large tendency to roll over. If the control surfaces of the airplane are not effective enough to counteract the airplane roll tendency, the pilot may lose control or, in a violent case, experience structural failure. (Langley Research Center, 2005, Chapter 4) The takeoff and landings of the new generation of jumbo jets compound the problems of severe tip vortices. During takeoff and landing, the speed of the airplane is low and the airplane is operating at high lift coefficients to maintain flight. The atmospheric conditions aloft were favorable for a slow rate of vortex decay. The wake vortex from Japan Air Lines flight 47 would have had an age of about 100 seconds, and flight 587 would have encountered the wake vortex at a time before vortex linking and rapid vortex decay. The predicted circulation of the wake vortex at the times of the apparent encounters would have been between 63 and 80 percent of the vortexââ¬â¢s initial strength. In testimony at the public hearing, the main author of the wake vortex investigation report stated that, even though his ork supported a wake encounter, the wake was ââ¬Å"nothing extraordinary. â⬠(NTSB, 2004, p. 57) Aircraft Pilot Coupling Aircraft pilot coupling (APC) was previously known as Pilot Involved Oscillation (PIO). An APC event is when the dynamics of the aircraft (including the flight control system [FCS]) and the dynamics of the pilot combine to produce an unstable pilot vehicle system. APC events can result if the pilot i s operating with a behavioral mode that is inappropriate for the task at hand, and such events are properly ascribed to pilot error. However, the committee believes that most severe APC events attributed to pilot error are the result of adverse APC that misleads the pilot into taking actions that contribute to the severity of the event. (Aeronautics and Space Engineering Board Commission on Engineering and Technical Systems National Research Council, 1997, p. 14) APC problems are often associated with the introduction of new designs, technologies, functions, or complexities. New technologies, such as FBW and fly-by-light flight control systems, are constantly being incorporated into aircraft. As a result, opportunities for APC are likely to persist or even increase. (Aeronautics and Space Engineering Board Commission on Engineering and Technical Systems National Research Council, 1997, p. 19) Flight 587 APC Event What the pilots of Flight 587 did not know was that the rudder controls on the A300-600 become increasingly sensitive as airspeed increases above 165 knots; this unique sensitivity creates adverse APC propensities primarily in the lateral axis. Flight AA587ââ¬â¢s APC event was triggered by an unexpectedly sensitive response of the rudder to an initial, single pedal input by the pilot during a wake vortex encounter. Due to the unique characteristics in the aircraftââ¬â¢s flight control system design, the pilot became caught in an adverse APC/pilot involved oscillation mode as he attempted to counter the effects of that input. Specifically, after making a control wheel input followed by a rudder input intended to achieve a desired aircraft response, the over-sensitivity of the rudder control system induced the pilot to make additional, essentially cyclic, corrective rudder inputs as he attempted to stabilize the aircraft. There were many different aspects that mirrored Jackson and American nationalism. Many factors, including his personality, his policies, his actions, and the way he mirrored American nationalism changed America into what become less of an aristocracy and more of a democracy to benefit the common man. Jackson was a man of humble background. In his time, a man that was born in a cabin was looked upon highly, and some of those with more noble upbringings actually apologized for not being born in more humble surroundings. Jackson was a brash, strong-willed man. He first got his fame in 1815 when he defeated the British Army at New Orleans with his untrained militia. Then in 1818, he violated Spanish territory without any authorization and hung 2 Spanish subjects for supposedly aiding some of the Indians that were being hunted. He lost the election for presidency in 1824. He finally got his presidency in 1828. Most of his votes came from the West and South. When he was in office, he made it clear that he would get his way. He was labeled "King Jackson the First" by some and he expanded the power of the President. He supported a strong national government and used his power to get what he wanted. He vetoed 12 times in his 2 terms in office. His presidency was one of violence, and a sort of monarchy rule. He was truly a man of the people. He also ignored many of the decisions made by the Supreme Court. Another thing was the Jackson had a strong personality and was well liked they supported him because he was a man of the people. Effect of PRF After Mandibular Third Molar Surgery Post extraction wound healing and physiologic sequelae of third molar surgery can significantly affect the patientââ¬â¢s quality of life2. Various methods have been suggested to improve extraction socket healing and to minimize the postoperative sequelae after third molar surgery4,5. PRF is a second-generation platelet concentrate which is known to stimulate wound healing by releasing growth factors directly to the wound5,8,16,17. Recent studies demonstrated the PRF membrane has a very significant slow sustained release of key growth factors for at least 7 and up to 28 day13. Studies on PRF efficacy in enhancing wound healing have yielded differing results and also there is a divergence of opinion on the activity of PRF. Thus, there is the need for more studies to explore its use in oral and maxillofacial surgery. This study was therefore perceived to determine the effect, if any, that PRF has on postoperative sequelae and enhancement of bone healing after mandibular third molar s urgery. Patients and Methods The study sample consisted of 15 young adult volunteers 18 to 22 years old presenting with impacted mandibular third molars bilaterally and in similar positions. Patients using systemic drugs or presenting a medical history positive for any systemic pathology or a history of hypersensitivity to any component used in the methodology were not included. Each patient participated as a volunteer after signing an informed consent form on the study for extraction of impacted right and left mandibular third molars. Ethical clearance was obtained from the Ethics Committee prior to the commencement of the study. The following demographic information was collected: age, gender, occupation, marital status, and educational level. Furthermore, the position and type of impaction were recorded. The two operated sides in each patient were randomly divided into 2 study groups: extraction of an impacted mandibular third molar at first side whose socket was filled with blood clot and wound sutured (cont rol group), and extraction of an impacted mandibular third molar on the other side whose socket was filled with PRF gel and wound sutured (PRF group). A 10-point visual analog scale (VAS) with a score of 0 equals ââ¬Å"no painâ⬠and ten equals ââ¬Å"very severe painâ⬠was used to assess pain. Facial swelling was evaluated using a modification of the method described by Schultze-Mosgau et al9, and this entailed measuring the distances from the tragus to the oral commissure and tragus to the pogonion. The arithmetic sum of the two measurements was used to determine the facial swelling at the time point. The percentage facial swelling was calculated from the difference of the measurements made in the preoperative and postoperative periods divided by the value obtained in the preoperative period and multiplied by 100. (Swelling Postoperativelyââ¬âSwelling Preoperatively) /Swelling Preoperatively X 100 Digital radiographs were used to evaluate changes in radiographic bone density for each extraction site. Three blinded dental professionals working independently evaluated all radiographs. RVG software was used for radiographic analysis. Radiographs were evaluated by three blinded dental professionals. The radiographs were assessed by obtaining the average density of three independent readings of the 3rd molar extraction socket sites. This was compared to the average of three density outlines of the adjacent tooth. When all radiographs for a patient were assessed, they were normalized to the original radiograph and the same untouched adjacent tooth. The baseline socket average was then subtracted from the normalized average for each tooth extraction socket at the different time points. The final, normalized socket value differences for each PRF-treated and non-PRF treated site for the various time points were compared. All radiographs were taken by a blinded, certified x-ray dental assistant. They were taken immediately post-operatively and at the following time after the day of surgery: 4th week, 8thweek and 12thweeks. Preparation of Platelet-Rich Fibrin PRF was prepared according to the technique described by Choukroun et al1. Twenty minutes before starting surgery, 10 mL of venous blood was collected in a sterilized dry, neutral glass tube without an anticoagulant. After immediate centrifugation at 3000 rpm for 10 minutes, the platelet-poor plasma, which accumulated at the top, was discarded. PRF was removed approximately 2 mm below its connection to the red corpuscle beneath to include remaining platelets, which have been proposed to localize below the junction between PRF and the red corpuscle After 10 minutes, the gel was formed, and autologous PRF was applied immediately in the socket in the PRF group. Surgical Procedure- To standardize the surgical operations, all patients were operated by the same surgeon. The mandibular third molars of which the extraction sockets would receive PRF treatment were selected randomly before surgery and these molars were operated on first. After tooth extraction, the surgical area in the PRF group was isolated with gauze and the socket was filled with PRF gel. In the control group, the socket was not filled with PRF. The flap was repositioned by an interrupted suture with 3-0 silk suture. The patients received oral and written postoperative guidance, and follow-up was scheduled according to the study periods. Because only the bilateral mandibular third molars similarly impacted were selected for the study, there was no significant difference in the time and surgical trauma associated with surgical extractions. Immediately after the procedure, details of each procedure were recorded, including the duration of surgery in minutes (from the first incision to insertion of the last suture), and any intraoperative complications. Patients were then recalled at 1, 3, 7, and 14-day postoperative intervals. During such visits, data were recorded for postoperative pain,facial swelling and any untoward complications like infection and alveolar osteitis. Sutures were removed 7 days after surgical operation. Patients were also recalled at the 4th, 8th, and 12th week postoperatively for radiographic bone healing assessment. Results The mean age of the sample population was 21.40(range 18 to 35 years). There was no statistically significant difference in the age, gender, and type of impaction between both groups. In both groups, the mean postoperative pain score (VAS) was highest at postoperative day 1 and gradually reduced over the following 14 days. The mean postoperative pain score (VAS) was higher for PRF group than control group at all-time points when compared with the control group and was not statistically significant (P The percentage facial swelling for the PRF group relative to baseline value was 2.55%, 2.22%, and 0.28% on postoperative days 1, 3, and 7 respectively, whereas it was 1.86%, 1.5%, and 0.29%, respectively, in the non- PRF(control) group for the same period. The percentage facial swelling was highest at postoperative day 1 and gradually reduced over the following days for both groups. The mean percentage swelling was also higher for the PRP group at all-time points when compared with the control. However this difference also was found to be not statistically significant. The mean bone density score was higher in the PRF group than the non-PRP group over a 12-week period; and the differences were found to be statistically significant. The results of the present study clearly show that the application of autologous PRF in surgical wounds after tooth extraction allows acceleration of bone healing, as indicated by data with statistical differences (p DISCUSSION This prospective study evaluated the performance of PRF gel when applied to fresh extraction sites. Third molar extractions are often used as a measurement tool for comparing treatments because they are usually performed electively on a younger population that do not present with significant medical problems (e.g., systemic pathologies, multiple medications). Therefore, this study is especially relevant for healthy 18 to 40 year old patients. This study evaluated the effect of PRF gel on postoperative pain, swelling and bone regeneration potential on third molar extraction sockets. The mean postoperative pain score (VAS) and swelling were higher for the PRF group at all-time points as compared with the control group, but the differences obtained were found to be statistically non-significant. This suggests that topical application of PRF may not improve the postoperative sequelae after third molar surgery. Choukroun et al5 in a case report found reduced pain and better mouth opening when topical PRF gel was used in molar extraction sockets ,in contrast to this we observed increased pain and swelling when topical PRF gel was used in 3rd molar extraction socket . Intra-oral digital radiographs taken of the individual surgical sites revealed that the effects of PRF were significantly beneficial (P 6,similarly reported that the PRF induced complete bone filling of a residual cystic cavity in 2 months 2 weeks, a much shorter period than 6 to 12 months of physiologic healing. Choukroun et al7 in a case series on maxillary sinus lifting operations, 3 cases were treated with PRF and freeze-dried bone allograft mixture and 6 cases with freeze-dried bone allograft alone. Histomorphometric results of this study showed that PRF and allograft mixture accelerated bone regeneration, allowing implant placement in 4 months after maxillary sinus lifting procedure. Furthermore, the amount of newly formed bone was equivalent to that achieved with an allograft alone 8 months after surgery. Diss et al10 reported promising results after placing PRF instead of bone graft under the sinus membrane during a closed-sinus lifting technique and demonstrated that an ave rage of 3.2 mm bone gain could be obtained in the sinus after 1-year follow-up. However the results of our study does not correspond with the study from Gà ¼rbà ¼zer et al17 who reported scintigraphically, that platelet-rich fibrin might not bring about promoted activity of osteoblasts in impacted mandibular third molar sockets in 1 or 4 week after extraction. Accelerated bone formation observed is in contrast to the drop in bone density seen at the control site before bone formation began to take place. It took approximately 6 weeks for the control sites to reach the same bone density that the PRF-treated site had reached by 4 weeks. The PRF-induced acceleration in bone formation may be due to the presence of bone morphogenetic proteins (BMPs) in PRF that stimulates mesenchymal stem cells to begin osteoblast differentiation and subsequent calcification12,14,18. The immediate start of bone formation seen with PRF treatment is of clinical relevance because it is the initial 2 weeks following bone manipulation in oral surgery that are important in preventing infection, loss of the blood clot and/or dry socket formation. Digital panoramic radiographs might have been a better choice than digital periapical radiographs, as the 2 surgical sites would be represented on 1 film, eliminating the need for normalization between the sites. The disadvantage of digital panoramic radiographs is that these films have a 20 to 25% distortion factor because it is an extra-oral film, as opposed to the intra-oral individual (periapical) radiographs.The digital periapical radiograph was chosen over the panoramic radiograph due to the lower distortion, which results from being in close proximity to the site being evaluated. The CT scans might have exhibited greater differences between the PRF treated sites and the control sites, had they been obtained. Similarly Digital substraction radiography and histomorphometry have provided more sensitive methods of assessing bone healing but are quite invasive and expensive especially in a resource limited environment. Conclusion The results of the present study suggest that topical application of autologous PRF gel has a beneficial effect on the osseous healing of extraction sockets after third molar surgery but may not minimize the postoperative sequelae after 3rd molar surgery as its use was found to be associated with increased postoperative pain and swelling . However, a larger sample size in a multicenter study may be necessary before its routine use in extraction socket can be justified
